Once you have been referred by your GP, you will then be contacted by a representative from One You Lincolnshire. They will ask you a series of questions in regards to your health and exercise levels to ensure you are eligible for the programme. If you are eligible, your details will then be passed on to our Exercise on Prescription team where they will then contact you to ask you a series of questions and book your initial consultation.
