This can be accessed through your Health Professional or you can self-refer via the One You Lincolnshire (OYL) website. Please note, sometimes self-referral may pause, due to applications being at capacity.

Unfortunately, we do not accept repeat referrals unless a 2-year period has lapsed, or referrals from someone who has, or had, a Magna Vitae membership within the past 12 months.
